PVL Team Captains Agreement Form MUST be signed/completed and turned in on the FIRST day of your game. (NO LATER!)
Roster Form MUST
be finalized of no more than 12 players max on the Roster. All players
must sign/complete on Tuesday, September 6th and handed in at the
PVL table before your scheduled game.
For
every match (3 sets of games) after Sept. 13th, if rosters are not
COMPLETED and handed in, then (1) set of each match will be forfeited.
Meaning score of (1) set will be "0" points other team will get
21.
Parental Consent Form be signed/completed and turned in on the FIRST day of your game. (NO LATER!)
New PVL Online Payment process. NO CASH OR CHECKS WILL BE ACCEPTED!:
($100) Initial Late League Fee to reserve your team's spot is due by Sept 6th - Please use Online payment method below to register your team, add Captain's Name and Team's Name under Special Instructions-this is to track how many teams are playing this year.
($300) Final League Fee due date is Sept. 13th - Reminder to again add Captain's Name and Team's Name under Special Instructions-this is to track how many teams are playing this year.
A
($50.00) late fee will be added every week for (3) weeks of games on
top of the $300 final league fee due Sept. 13th (max $150.00 late
charges will apply).
If payments are not paid in full, then team will be disqualified from the league as of Sept. 27th.
